Keep this card with your identification so police or first responders can call someone to care for your pets in the event of an emergency.
Help My Dog Authorization Card
If you travel with your dog, this card tells first responders who to call in case of an emergency. It also give explicit consent (along with the Pet Care Authorization Form) for a veterinarian to treat your dog in the event you aren’t able to give consent.
Forms
Pet Care Emergency Authorization Form
Provides written consent to veterinarians to provide life-saving emergency care to your dog if you are unable to give verbal consent. Keep a copy in you car at all times!
Temporary Treatment Authorization Form
This form authorizes another person to make medical decisions for your pets. Complete this form whenever you leave your pets with someone else (a pet sitter, day care, boarding facility, etc.). Make sure to give them this form as well as a detailed written medical history including any medications your dog takes.
